Client Agreement for Hypnotherapy Sessions

Welcome 

Thank you for choosing to work with me. As part of my training with CPHT, I am offering hypnotherapy sessions to build experience and complete my case studies. This agreement outlines how we’ll work together, how your personal data is managed, and what you can expect. 

1. Session Details 

- Sessions are conducted online via [Zoom/Skype/etc.]. 
- Each session will last approximately 50 minutes. 
- As I am currently in training, sessions are offered at a reduced rate (half price) until full qualification is achieved. 
- Sessions may be audio recorded for supervision purposes (see Section 4). 

2. Confidentiality 

- Everything discussed in sessions remains confidential, unless I am concerned about your safety or someone else’s. 
- As a student, I may share anonymized aspects of your sessions with my supervisor for educational purposes. 

3. Cancellations & Rescheduling 

- Please give at least 24 hours' notice if you need to cancel or reschedule. 
- As this is a voluntary offering, I kindly ask you to commit to sessions with respect for the time involved. 

4. Data Protection (GDPR) 

- Your personal data will be handled in accordance with the General Data Protection Regulation (GDPR). 
- I am registered with the Information Commissioner's Office (ICO) under registration number: CSN6844895 
- Data collected may include: 
  * Your name and contact details. 
  * Notes from sessions (kept securely and confidentially). 
  * Any forms or questionnaires you complete. 
- Your data will not be shared with third parties and will be securely stored for a period of [e.g., 8 years] in line with industry guidance. 
- You have the right to: 
  * Access your personal data. 
  * Request correction or deletion. 
  * Withdraw consent at any time.

6. Disclaimer 

Hypnotherapy is a complementary therapy and does not replace medical advice. You are advised to consult your GP or relevant healthcare professional regarding any medical or psychological condition.

By proceeding with hypnotherapy sessions, you confirm that: 
- You understand the nature and scope of the sessions. 
- You consent to the collection and use of your personal data as outlined above. 
- You are over 18 years of age (or have parental/guardian consent if under 18).
